Charge Authorization

You may give your charge card number when you register, but your card will not be charged and your payment is not complete until we receive a charge authorization from you. AFTER you have your student id number, please complete the form.

If the authorization is not received by your payment due date, your reservation will be dropped.

Sending this form complies with California Civil Code 1633 as an electronic signature and has the same legal force as a physical signature on a charge slip.
